After an individual has been found eligible for home and community-based services, the funding request has been approved, and a service provider has been selected, the Designated Agency (DA)/Specialized Service Agency (SSA), or the Independent Support Broker if self/familiy-managing, along with the individual and his/her team, will develop a plan for supports. There are two levels of funding committees for home and community-based services (HCBS): The designated agency Local Funding Committee, and the State Funding Committee.
Any individual who believes he or she has a developmental disability, or is the family member or guardian of such a person, may apply for developmental disabilities (DD) services.
The individual must apply for Medicaid, if they are not already a Medicaid recipient, as he or she needs to be approved for Medicaid prior to receiving any DD services.
