After an individual has been found eligible for home and community-based services, the funding request has been approved, and a service provider has been selected, the Designated Agency (DA)/Specialized Service Agency (SSA), or the Independent Support Broker if self/familiy-managing, along with the individual and his/her team, will develop a plan for supports. This plan is called an Individualized Support Agreement (ISA). The ISA is an agreement between the individual and his/her service provider regarding how the individual expects to be supported to meet the identified needs. It outlines what the individual hopes to gain from the supports. The agreement is flexible and personalized so that an individual and his/her team can be creative in how supports are designed. It also addresses how to help ensure the individual's health and safety.
Once an individual has begun receiving services, his/her needs and authorized funding limit are re-assessed annually or when the person's circumstances change. If additional services are needed, the individual may request services from their service provider.
Individual Support Agreement (ISA) Guidelines
