The DA/SSAs are required to complete an Emergency Fact Sheet for each individual they serve with our funding, and have it available in the individual's file but also accessible whenever the individual is with agency paid staff. It list all important contact and medical information on in one document that can easily be presented to emergency responders and/or medical personnel in an emergency situation.
